Opportunity
PCRS is looking for an experienced Primary Care Registered Nurse to work at the Foundry Surrey location. With a passion for youth health, they will join an interdisciplinary team to deliver client-centered medical care to youth ages 12-24 in an integrated youth health clinic. Foundry Primary Care professionals see youth for longitudinal, episodic, or specialized care. Care areas include but are not limited too: sexual health (STI and contraception management), gender affirming care, substance use, health, mental health, physical assessment and screening of general health conditions. Reporting to the site manager, the registered nurse assesses, identifies, plans, implements and evaluates the nursing care required to assist youth in meeting their physical, social, spiritual and psychological needs. The Registered Nurse works within an interdisciplinary clinical framework in accordance with the BCCNP Standards of Practice and is consistent with the mission, vision and values and established protocols of Foundry Centres.
Responsibilities
- Collaborate with physicians and/or nurse practitioners, the inter-professional team and other services to develop, implement and evaluate comprehensive care plans and interventions for youth and their families using systematic assessment, best practices and evidence-based standards, involving the youth and, where appropriate, the family in the context of their community.
- Participate in the intake, assessment and care planning for youth.
- Maintain participant health records and files.
- Incorporates individual and/or group teaching into the care plan and nursing interventions and/or makes referrals to other identified resources.
- Support the with medication management system and dispense medication according to scope and role.
- Assist in providing basic care and medical support to youth on-site.
- Provide integrated services for clients. Acts as a resource for primary care, gender affirming care, mental health and substance use issues.
- Promote collaboration between staff and families to highlight client concerns such as access to primary care, education, employment, housing and income support. Encourage self-care; provide access to interventions; foster a warm and non-judgmental environment; provide guidance, support and information on situations.
- Assist with navigating issues such as informed consent, confidentiality and developing linkages and partnerships with other facilities, agencies and community resources to best meet patient needs.
- Provide basic care and support to patients, including monitoring vital signs, documenting detailed patient information, and assisting physicians and nurse practitioners with care and treatment.
- Perform other related duties as required.
Qualifications
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Two years’ recent, related experience ideally working with youth and young adults in a primary care and/or mental health/substance use setting.
- Current CPR or BLS Certificate.
- Successful completion of the BCCDC Immunization certificate.
- CRNBC Sexually Transmitted Infection (STI) Management, and contraception management (CM) certification, or willingness to enroll and complete the training.
- Criminal records check including the vulnerable sector
- Tuberculosis and medical clearances
- Class 5 Driver License and submission of a Driver’s Abstract
- Physical ability to carry out the duties of the position.
Additional Assets
- Comprehensive knowledge of the broad range of issues pertaining to youth including sexual health, gender affirming care and mental health and substance use among adolescents, young adults, and their families.
- Demonstrated knowledge and application of trauma-informed and culturally safe principles of care.
- Ability to work effectively with others in a multicultural, interdisciplinary environment, independently and as part of an interprofessional team, including understanding the frameworks of other disciplines
- Ability to make effective referrals in a collaborative manner, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community partners.
